Has anyone participated in an Affiliate Program?
If so;
What type of product was it?
Was it exclusive to internet or was it door to door?
What amount did they pay?
Was it based on production levels?
Was there an entry fee?

I was a member f the ebay affiliate program and was making pretty decent money with my websites. The website that was making the most money was http://classicvehicleauction.com; however, when they changed their affiliate structure to pay per click rather than a commission the income became negligible.
Having an affiliate program that is exclusive to the internet is probably the easiest and I think that affiliates are more likely to trust that their efforts will be rewarded because both you and them can track the sales they bring you via affiliate tracking software. In contrast, depending on how they provide you with a contact (off the internet) they may have difficulty establishing whether or not it led to a sale.
The amount an affiliate program pays depends on how aggressive they are with respect to competitors. I think 25% is a good number for what you are offering.
Offering an additional percentage for production and second tier affiliates is a good idea.
Asking for a fee would be a bad idea, unless you want to wholesale the services so they can sell it themselves - making the price slightly higher than what they would pay for buying the service from you direct so there is no incentive to claim they will wholesale the services just to get a better price (common with host providers).

I have done quite a bit of affiliate marketing in the past two years.
The best programs are usually through product consolidators like Clickbank and Commission junction.
The trend right now is moving toward shippable products. Ebooks and services are getting so competitive that buying traffic for them is not working very well.
If you bid on a specific product though…chances are the person looking for that product is looking to buy, not just get information.
