I assume the law applies to Joe Schmoe landlord as opposed to someone who has formed an LLC. I suppose that would be the exception to the law.
By the way, do you recommend forming an LLC?

You should contact a lawyer for advice, but I would think that one rent house is not a huge push. But after the second one place them in LLCs. I do 3 houses per LLC that limits any issues with one house to her 2 sisters and not my whole empire. The names of the LLCs have no connection to me like Bills houses. I pay all the bills in the name of the LLC and the tenants pay rent checks to the LLC.
Not sure about that. I own rental property in VA and I’m giving out any info like that. My property management companies handle all that stuff.
I think what you are refering to is in the lease you need to include a physical location for payments to be sent, place where tenants can reach you, etc. This is pretty standard in any lease (or contracts for that matter).
forming an LLC is easy. understanding it and running it correctly is a whole different ball game.
get legal advice. pay for it. it’s worth it. just don’t pay too much. do some research on your own. formulate good questions. ask them on here first and maybe people can help you with them.
unfortunately, no one can just give you the questions to ask. you have to do that on your own because it’s your mind that needs the info. FAQ’s can be found also in Nolo books. they’re great. i highly recommend them.
