Bird dog fee agreement forms?

As a new bird dog I am looking for a copy of a form that I could use with the investor that I am going to be bringing the leads to. Something that shows in writing that the investor has promised me the agreed fee for lead and also the agreed on fee if he purchases and sells the property. Seems like a document like this would be in my best interest.
